If you are required to supply proof of insurance when you rent your fields.  Please download the Blank Field Insurance form.  Fill out the pink shaded area and than email it to the President.  He will get it processed and send back you proof of insurance when he receives it back from the insurance company.

The Registration for the Minnesota and Wilson Cups is done on-line .  See the LINK on the Home Page.

Both Cups are run by the MASL.  You can find more information about the two Cups on the Tournaments page.  For questions please send them to the MASL Tournamant Director.  
 

The registration for NON-MASL teams is done by requesting a registration form from The MASL Tournament Director.  Send request to President@masl.org

This report is to be filled out and sent to the President of the MASL.

It must be submitted within 60 days of the injury in order to submit to the insurance company.
